Ethics Tip Line Report Form

In order to combat all forms of waste, fraud and abuse that result in lost money, time and productivity as well as create uncomfortable working conditions, the Liquor Control Board has established a reporting process for complaints. This general form can be used to report a variety of complaints, some of which are described in more detail on our FAQ page.

Please complete the questionnaire below as thoroughly as possible. If you have any questions or would like to speak to someone regarding the reporting of possible waste, fraud or abuse, please call 855-693-2062.

In an effort to encourage reporting and to maintain confidentiality, your report may be made anonymously. However, please note that if you are not using your name, you will be provided with a randomly generated identifier that will be used to identify you as the reporting party in order to claim any possible reward or when making an inquiry regarding the progress of the case. Please be advised that the potential recovery reward is equal to 10% of confirmed losses up to a maximum reward of $1,000. Your identifier number will be automatically generated with a time and date stamp upon submission of the form. Please copy or print out the confirmation page and retain the identifier for your records.

Description of Incident: (Describe as clearly as possible the incident(s) you are reporting. For example: Who did what? Where was it happening – in the store, workplace or nearby? How do you know of the incident? Why do you believe that it is fraudulent? Remember, if you are remaining confidential the only information we will have in order to investigate is what you supply us now. It is important that you give us as much detail as possible)
Please be advised that the Pennsylvania Whistleblower Law [43 P.S. §§ 1421-1428] provides protection from discrimination and retaliation to any person who witnesses or has evidence of wrongdoing or waste while employed by a public body (e.g., state, county or city agency, department, division or council, etc.) and who makes a good faith report of the wrongdoing or waste, verbally or in writing, to one of the person’s superiors, to an agent of the employer or to an appropriate authority. Under this law, no employer may discharge, threaten or otherwise discriminate or retaliate against an employee regarding the employee’s compensation, terms, conditions, location or privileges of employment because the employee, or a person acting on behalf of the employee, makes a good faith report or is about to report, verbally or in writing, to the employer or appropriate authority an instance of wrongdoing or waste.
